Surgical Guides

According to the associated literature and recent advancements in implant dentistry, three categories are revealed when it comes to the fabrication of surgical guide templates, based on design concepts like nonlimiting design, partially limiting design, and completely limiting design.

Assisting in diagnosis and treatment planning as well as facilitating proper positioning and angulation of the implants in the bone are the purposes of fabrication of surgical guide templates.

A diagnostic tooth arrangement through a diagnostic waxing, a trial denture teeth arrangement, or the duplication of a pre-existing restoration is needed when fabricating surgical guide templates.

Nonlimiting designs only provide an initial indication to the surgeon as to where the proposed prosthesis is in relation to the selected implant site. This design indicates the ideal location of the implants but it fails to emphasize on the correct angulations of the drill, therefore, it allows too much flexibility in the final positioning of the implant. These templates may serve as imaging indicators during the surgical phase of implant.

The partially limiting design allows only the first drill for osteotomy, and the remainder of the osteotomy and implant placement is finished freehand by the surgeon. Techniques based on this design philosophy basically follows the initial fabrication of a radiographic template, which is then converted into a surgical guide template following radiographic evaluation. This design is employed in single implant therapy or short-span implant-supported prostheses.

Completely limiting design restricts all of the instruments used for the osteotomy in a bucco-lingual and mesio-distal plane. Morover, the addition of drill stops limits the depth of the preparation, and thus, the positioning of the prosthetic table of the implant. This includes cast-based guided surgical guide, and computer-assisted design and manufacturing (CAD/CAM) based surgical guide.

Safety Concerns

Sterilization Methods

Not only the proper equipment is need for an effective sterilization process of reusable dental instruments, also proper methods to ensure an efficient result, and prevent medical staff suffering an accidental injury.

Use of automated methods like instrument washers, thermal disinfector, and ultrasonic cleaners instead of manual scrubbing, increase productivity, improve cleaning effectiveness, and decrease work exposure to blood and body fluids.

The first step must be to discard single-use sharps such as needles, blades, orthodontic wires and glass into sharps containers in the dental treatment room, while items without risk can be thrown into the trash.

Special handling should be used to transport the reusable items in a tray or cassette to the cleaning and sterilization area to prevent accidental injury with contaminated instruments. Heavy-duty gloves must to be used always. Mask, eyewear and gown should always be worn while cleaning if manual scrubbing is necessary. Using any automated method minimizes handling of instruments.

At America Dental we use ultrasonic cleaner and autoclave cleaner for an effective and efficient sterilization. Ultrasonic cleaner uses a process called cavitation, by oscillating bubbles to remove debris from the instruments with sound waves. A rack or basket fitted to the machine holds instruments suspended in the ultrasonic bath. It is important to avoid overloading the ultrasonic device, and do not lay instruments directly on the bottom of the machine because it can causes damage to both the ultrasonic device and the instruments.

When using the autoclave cleaner must be taken into account that this machine is a mild acidic salt cleansing compound formulated for removal of water deposits, oxides and other sediments, and it also might absorb some bacterial matter.
